Prostate cancer: what the long-term follow-up of PEACE-1 shows

The PEACE-1 trial confirms a survival benefit for some metastatic cancers. Population, median and risks: key points for understanding the results.

In some prostate cancers that have already metastasised by the time of diagnosis, combining several treatments improves observed long-term survival. Updated results from the PEACE-1 trial, presented at ESMO in October 2026, support this strategy. But interpreting them calls for one essential clarification: a difference between two groups does not predict how long an individual will live.

In the subgroup treated with hormone therapy and docetaxel, adding abiraterone increases median survival from 4.4 to 6.2 years. The difference is 1.8 years. The result is important; it does not mean a cure or an identical gain for every patient. Understanding the population studied and what this measure means makes it possible to assess its significance without turning hope into a promise.

Which patients the study concerns

The authors’ abstract published on the European Association of Urology’s platform concerns metastatic cancer from the outset, which is still sensitive to hormone suppression. Metastatic means that the disease has spread beyond its original site. “From the outset” specifies that this spread is present at diagnosis.

PEACE-1 is a phase 3 randomised trial. Median follow-up has now reached 7.7 years. The analysis of treatment combining androgen deprivation, docetaxel and abiraterone covers 710 participants who received the androgen deprivation-docetaxel backbone.

These criteria mean the result cannot be applied directly to all prostate cancers. Localised disease, a recurrence after initial treatment and disease that has become resistant to certain hormone-based approaches are not interchangeable situations. The name of the organ alone is not enough to determine treatment.

Why combine these treatments

Hormone therapy acts on the hormonal stimulation on which some tumour cells depend. Docetaxel is a chemotherapy drug. Abiraterone is another intervention targeting hormone production; it is therefore not simply a second chemotherapy drug.

The European Medicines Agency’s information sheet on Zytiga, which contains abiraterone acetate, explains that this substance inhibits an enzyme involved in testosterone production. It describes use in combination with prednisone or prednisolone in specific situations. The term “triple therapy” summarises the main anticancer components; it is not a complete list of a prescription’s contents.

Combining several mechanisms is not automatically preferable in every situation. Its value must be demonstrated in the population concerned, then weighed against the risks and constraints. This is precisely the role of comparative trials, followed for long enough to measure the effects being sought.

A median is not an individual deadline

The National Cancer Institute defines median survival as the time from the chosen starting point at which half the patients in the group are still alive. It is a statistical marker, not a time limit.

A hypothetical example helps illustrate this: two groups can have the same median while showing different outcomes before and after that point. The median summarises a distribution; it does not tell the story of every patient’s journey. Saying that it increases therefore describes a collective benefit, without predicting what will happen to each person.

It is also important to distinguish overall survival from the time without disease progression. The first measures survival time; the second addresses a different question. Using these terms interchangeably could give the same figure a meaning it does not have. Here, the figures of 4.4 and 6.2 years refer to median overall survival in the subgroup described.

The authors also report estimated eight-year survival rates of 27.1 % with the doublet and 40.3 % with the triplet. These figures supplement the median. They should not be presented as a personalised probability, independent of clinical condition and treatment history.

A benefit that does not remove the need to monitor tolerability

Side effects cannot be summed up as “few risks”. The EMA lists high blood pressure, low blood potassium, fluid retention and abnormal liver enzyme levels among the common side effects of abiraterone. It also flags significant risks affecting, among other things, the heart or liver.

This information does not describe the exact frequency of each complication in the PEACE-1 analysis presented here. It serves as a reminder of why monitoring is part of treatment. A useful combination may require tests, adjustments and attention to other illnesses or medicines.

The decision therefore rests with the healthcare team, based on the individual’s medical record. An article about a trial is no reason to add, stop or change a medicine on one’s own. For someone concerned, the useful question to ask is how closely their situation matches that of the participants, and what benefits and constraints can be expected in their case.

What this follow-up adds, and what it does not settle

Long-term follow-up helps assess whether a benefit persists. It does not replace scrutiny of the methodology, uncertainty intervals and treatments received over time. The source used here is the conference abstract of the updated analysis, not a review of all the individual data.

This abstract states that Unicancer is the entity responsible for the study and names Johnson & Johnson, Sanofi and Ipsen among the funders. This transparency is information to keep alongside the results, without inferring from it alone that they are valid or invalid.

The documented progress is therefore limited in scope but concrete: in the population studied, treatment intensification is associated with better long-term survival. The most useful way to discuss it is to consider together this benefit, the limits to how widely it can be applied and the monitoring required. A figure can inform a medical discussion; it cannot replace it.
